The Million Hearts® Cardiovascular Disease (CVD) Risk Reduction Model
United Medical is pleased to announce its acceptance by CMS as one of 427 practices to participate in the Million Hearts Cardiovascular Risk Reduction research program.
The model is designed to promote CVD prevention, improve CVD outcomes, and accountability for costs among Medicare beneficiaries through risk assessment and risk management. Participating practices will use the American College of Cardiology/American Heart Association (ACC/AHA) Atherosclerotic Cardiovascular Disease (ASCVD) Pooled Cohort Equation 10-year risk calculator to stratify their attributed Medicare FFS beneficiaries and to develop risk modification plans based on beneficiary risk profiles. The format and implementation of the individual risk modification plans will be left to the discretion of each practice.
The model financially incentivizes providers to use the ACC/AHA ASCVD risk calculator to prevent heart attacks and strokes. The incentives structure identifies two types of payment- Cardiovascular Disease Risk Assessment (CVD RA) payment and Cardiovascular Care Management (CVD CM) payment.
Half of all selected applicants (n=360 intervention practices) will be randomly assigned to the intervention group with the remaining selected applicants assigned to the control group. If we are randomized into the control group, only 20 providers of our choosing will be eligible to participate in the model.
All payments for practices in the intervention group will be made from the Part B Supplementary Medical Insurance Trust Fund, and will not be subject to coinsurance or the Part B deductible. Practices randomized to the control group will receive a, per beneficiary cost-based payment to help cover the costs of preparing and transmitting the data to CMS.
We will be notified via email by May 9, 2016 of whether we have been assigned to the intervention or control group. Detailed instructions and timelines on preparing your practice for participation in the MH Model will follow after this date.

Advanced care planning including the explanation and discussion of advance directives such as standard forms (with completion of such forms, when performed), by the physician or other qualified health-care professional; first 30 minutes, face-to-face with the patient, family member(s), and/or surrogate.
